A Dietitian came into the pantry kitchen and asked for a sandwich for a patient the catering assistant asked which patient is it for and the dietitian stated the name of the patient, and the catering assistant then told the dietitian that this patient is on a mince moist diet and therefore could not have what the dietitian requested.

To Double check the Catering Assistant went to check the overhead bed sign and it was tick minced moist so therefore the patient would not be allowed a sandwich for their meal. If the patient had received this item this could have been fatal / choking incident.

As a result of this we Introduced overhead bed sign audits. We have trained our staff if the overhead bed sign is not filled out, we are not allowed to give the meal to the patient this is to ensure that the correct meal is given to the patient.
